Nurse Julianne Scott

Welcome Letter

Dear CSI Families,

Welcome to the 2015-16 school year! My name is Julianne Scott, and I will proudly serve as your child’s school nurse. Together with my amazing coworkers I will strive to keep your child safe, healthy, and in the classroom learning. One of a school nurse’s favorite tag lines is “Healthy Children Learn Better!”


I need your cooperation with this. By now I’m sure you have received a confidential health questionnaire on yellow cardstock paper. It is extremely important that you carefully complete and sign this form for each child in your family and return it as soon as possible. It is for Health Room use only and it contains the information I need to keep your children safe while they are at school. Students are not permitted to participate in PE or yoga class until this form is submitted. It is also critical that you notify me of any changes in contact numbers as soon as possible. I have to be able to reach an emergency contact during the day, should the need arise.

If your child requires prescription, non-prescription, or rescue medication, please let me know so that I can provide you with the proper authorization forms. Prescription and rescue medication requires a consent containing the prescribing physician’s signature and address. Over-the-counter medication (Tylenol, ibuprofen, Benadryl, hydrocortisone, etc.) requires a parent consent form. The school does not provide this medication; however, I have the capacity to store it in the Health Room for use throughout the year as needed.

One more thing to keep in mind . . . Children can have accidents. They may go down a wet slide, spill milk on themselves at lunch, get a nosebleed, have too much fun doing a science experiment, or have the occasional bathroom mishap. Please ensure that your child has a change of clothing (including undergarments) in his or her backpack or classroom cubby at all times. Children in grades 3K through 5K should have at least two changes. This allows me to care for the students without having to disrupt your day. Children will not be permitted to sit in class with wet, bloody, or otherwise uncomfortable clothing. Thank you in advance for your cooperation with this!
